JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the PATH/Street Outreach Manager, the Outreach Worker completes face-to-face assessments with clients in order to make the proper referrals to homeless shelters or to short-term case management, in accordance with agency policies and procedures.

EDUCATION and/or EXPERIENCE: High school diploma or equivalent and six to twelve months related experience; or a combination of education and experience.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Canvas streets, under bridges, encampments, meal sites, shelters and other congregate sites to identify potential clients. Perform face-to-face assessments of clients.
- Assist clients with obtaining food, hygiene kits, sleeping bags, and finding shelter or housing when possible.
- Distribute information packets on services available to clients.
- Assist clients in finding housing, medical and mental health assistance, obtaining legal assistance/benefits, AODA services and support, etc.
- Refer clients for screening and assessment and case managers as appropriate.
- Coordinate with case managers to assist in guiding clients to different resources for which they are qualified.
- Refer clients for assistance with obtaining social security and/or SSI benefits as possible.
- Complete the Encounter Form on client and input information/assessment into Wisconsin ServicePoint database.
- Transport clients to shelters when needed.
- Participate in weekly A.M. shift
- Complete special projects as assigned.
- Complete and maintain all required paperwork, records, documents, etc
