Job Purpose and Reporting Structure:
Under the direction of the HIM Supervisor, the HIM Technician is responsible for assembling and maintaining complete medical records in the Electronic Health Record (EPIC) system according to established procedures. The role includes filing and retrieving patient records, as well as scanning and indexing external documents into the Electronic Health Record using OnBase, in alignment with company policies and procedures.
Essential Duties and Responsibilities:
- Retrieve and file medical records in patients' charts according to filing guidelines within three days of receipt (date stamp all incoming filings with the date received).
- Respond to requests for records, prioritizing them as needed, and adhere to state and federal guidelines for Release of Information (both incoming and outgoing).
- Maintain and utilize various health record indexes.
- Prepare patient charts, ensuring all documents are filed correctly and in order.
- Operate computers to process, store, and retrieve health information.
- Pull charts for audits (e.g., insurance company, state, and medical audits).
- Analyze and review medical records for completeness and accuracy according to specified standards.
- Deliver incoming forms and prescriptions from external institutions to the appropriate provider for review.
- Follow up on requests for additional information from other institutions that have not been received in a timely manner.
- Compile, maintain, and review logs, reports, and statistical records; research records to locate health data as requested.
- Safeguard the confidentiality of medical charts/records and comply with all local, state, and federal laws related to medical records. Ensure compliance with all HIPAA regulations regarding the use, retrieval, storage, and sharing of medical records.
- Perform data entry and retrieval related to medical records.
- Participate in organization-wide quality improvement initiatives as requested.
- Assist in the production and revision of medical records-related policies and procedures.
- Adhere to clinic rules and regulations, including safety standards.
- Perform other duties as assigned.
- Complete special projects as assigned.
Qualifications:
- Education: High school diploma or GED required; college education preferred.
- Experience: Minimum of 2 years of experience providing excellent customer service, preferably in a busy healthcare setting.
- Preferred Skills: Experience with EPIC and familiarity with medical terminology.
