Job Purpose and Reporting Structure:
Under the direction of the TCM/CCM Program Manager, the Case Manager supports chronically mentally ill clients in achieving independent living within the community, following agency policies and procedures.
Essential Duties and Responsibilities:
- Conduct client interviews to complete assessments and develop comprehensive treatment plans, including case planning, referrals for services, ongoing monitoring, and service coordination.
- Assist clients with securing housing, medical and mental health services, legal assistance, and benefits. Support them in medication management, employment, training, education, financial management, and budgeting. Provide assistance with activities of daily living (ADLs), social support system development, and AODA services.
- Manage a caseload to ensure regular supportive contact and help clients navigate community resources.
- Handle client intake from initial referrals, meeting clients in various community settings, including hospitals, within established guidelines.
- Prepare discharge paperwork for clients who meet predetermined discharge criteria.
- Transfer clients when services are no longer appropriate or when more intensive services are required.
- Participate in county staff meetings regarding client transfers or discharges.
- Coordinate transportation for clients to and from appointments, including medical and mental health visits, shopping, and housing.
- Actively participate in departmental meetings and collaborate with other service agencies.
- Complete monthly billing for T-19 and county reimbursement of services provided to each client. Provide on-call coverage as scheduled.
- Ensure all required paperwork, records, and documents are completed and maintained accurately.
Education and Experience:
A bachelor’s degree in a relevant field is required, along with one to two years of related experience. A combination of education and experience may be considered.
