JOB PURPOSE AND REPORTING STRUCTURE: Under supervision of the CCS Program Manager, the Rehabilitation Specialist/Case Manager provides support and assists in developing functional living skills to consumers who are enrolled in the Comprehensive Community Service Program (CCS).
This includes, but is not limited to: housing, medication management, employment, money management and budgeting, education as well as social, spiritual and recreational services.
This position requires attention and responsiveness to a consumer’s changing needs, condition and goals for their participation in the CCS Program. The rehabilitation Specialist works in close collaboration with the consumer’s Care Coordinator, therapist(s) and other identified CCS service providers.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Work with CCS Care Coordinators and other team members to ensure clients receive appropriate and high quality care; including referrals for services, ongoing monitoring and service coordination.
- Complete home visits and appropriate services as needed and in accordance with clients Recovery Plan of Care (RPOC).
- Assist clients in the completion of daily living skills such as but not limited to; upkeep of their living environment, budgeting and overall money management, appointment scheduling and basic medication management.
- Assist clients in finding housing, medical and mental health assistance, obtaining legal assistance and benefits, in finding employment, training and education, ADL assistance, development of social support systems, AODA services and support, etc.
- Coordinate transportation for clients to and from appointments, to include medical and mental health, shopping, housing, etc.
- Maintain records of client contacts and complete ongoing documentation in electronic record database.
- May provide transportation to clients on occasion.
- Attend and actively participate in departmental meetings and meetings with other service agencies.
- Complete and maintain all required paperwork, records, documents, etc.
- Other duties as assigned
EDUCATION and/or EXPERIENCE: High School Diploma (required) and a minimum of 30 hours training/experience in recovery treatment and/or Certified Nursing Assistant training. Associate’s or Bachelor’s degree in Human Services field (preferred).
CERTIFICATES, LICENSES, REGISTRATIONS: Requires a valid WI driver’s license.
