Description

JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Director of Homeless Services, the Outreach Worker completes face-to-face assessments with clients in order to make the proper referrals to homeless shelters or to short-term case management, in accordance with agency policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Canvas streets, under bridges, encampments, meal sites, shelters and other congregate sites to identify potential clients.
  • Perform face-to-face assessments of the clients.
  • Search out new locations where clients can found.
  • Assist clients with obtaining food, hygiene kits, sleeping bags, and finding shelter or housing when possible.
  • Distribute information packets on services available to clients.
  • Assist clients in finding housing, medical and mental health assistance, obtaining legal assistance/ benefits, AODA services and support, etc.
  • Refer clients for screening and assessment and case managers as appropriate.
  • Coordinate with case managers to assist in guiding clients to different resources for which they are qualified.
  • Refer clients for assistance with obtaining social security and/or SSI benefits as possible.
  • Complete the Encounter Form on client and input information/ assessment into Wisconsin Service Point database.
  • Transport clients to shelters when needed.
  • Participate in weekly A.M. shifts.
  • Complete special projects as assigned.
  • Complete and maintain all required paperwork, records, documents, etc.
  • Attend necessary trainings and meetings.
  • Report any problems or service requests for Outreach vans.
  • Network with other agencies and collaborate with organizations and maintain working relationships.

EDUCATION and/or EXPERIENCE: 

Requires four years of high school and six to twelve months related experience; or a combination of education and experience.
